I had a customer send me that exact same picture last year. I purchased frames from a regional wholesale framing industry supplier I do business with, then cut my own hardboard backer and applied the Blue JDS leatherette sheet material to it and laser engraved silver border rectangles that identified where the perpetual plate placements should be. I do possess all of the standard framing tools to assemble and finish off the backs of the frames professionally. Now, each year I can send the customer the sticky backed perpetual plates they need and they add them to their plaques. This customer ended up ordering 18 plaques, one for each of their locations.
Here is a link to a supplier of some of the small framing tools you would need. These are good things to have in your creative tool box.
