I do, but honestly the couple times there's been an issue, I've replaced the item anyway. It just feels wrong not to, and no matter what a customer will of course be very upset whether they signed something or not. I think the agreement is moreso just for peace of mind knowing I'm not technically responsible, for instance if it's a very expensive item, or a one-of-a-kind/antique/etc., and the customer is made aware that there's always a non-zero percent chance something goes wrong.

Subject: Hold Harmless Agreements for Items Dropped off to Engrave
Does anyone ask customers to sign a hold-harmless agreement or waiver for accidental damage when they drop off items for engraving?
