    I also checked on Awards Etc, home page and contact page but it does not have an email or contact numbers.

    We were also interested in purchasing blanks from them.

    Yes,  this is a PlaqueMate plate.  Plaquemates the company and assests were acquired by Highest Honor.  The Plaquemates catalog of plates are still available.  Just email me to let me know which sizes you may need.
