That appears to be a custom shape and size.  You could have it made by Blue Ridge Mountain Woodcrafts or Claytex Trophies.  I'm not sure what quantity you are after.

Megan, We did something similar looking and cut black acrylic from JDS and also purchased hanging supplies from them that are in the sublimation area of the catalog. We did a standoff between the acrylic and sublimated plate to give it some depth.

hi Meagan, it looks to me like this is a tile plaque. An 8 inch sublimable tile set inside of a routed out plaque.
