The website shows they have the plates. In fact, it looks like they have about 82,000 of them. The header plate may be what they don't have but that wouldn't be difficult to make in-house if you have a laser. You could also get a one that may be close that they do have in stock.

JDS has the plaque and the plates to make that exact plaque. Why not order in the components and assemble yourself?

You can make your own header plate or use one of the10" long options they have such as ZX74 or ZX69.
